Peran Rasio Neutrofil Limfosit dan Platelet-Neutrofil terhadap Limfosit pada Pasien Tersangka Karsinoma Kolorektal di RS dr. Cipto Mangunkusumo = The Role of Neutrophil Lymphocyte Ratio and Platelet-Neutrophil to Lymphocyte Ratio in Patients with Suspected Colorectal Carcinoma at dr. Cipto Mangunkusumo Hospital.

Karsinoma kolorektal (KKR) merupakan penyakit kanker terbanyak ketiga dan penyebab kematian kedua terbesar di dunia. Tingginya angka mortalitas KKR akibat pasien datang pada stadium lanjut dan terlambat diberikan terapi. Kolonoskopi sebagai baku emas penegakan diagnosis KKR bersifat invasif, mahal, dan tidak nyaman untuk penderita. Pada pasien KKR, inflamasi kronis di usus berperan penting dalam proses karsinogenesis. Parameter hematologi seperti leukosit, neutrofil, limfosit, dan trombosit yang merupakan marker inflamasi berhubungan dengan progresivitas KKR. Rasio Neutrofil Limfosit (RNL) dan Rasio Platelet-Neutrofil terhadap Limfosit (RPNL) banyak diteliti sebagai marker prognostik KKR. Penelitian ini bertujuan untuk mengetahui peran RNL dan RPNL sebagai paremeter deteksi KKR dan kombinasi keduanya untuk memprediksi terjadinya KKR. Desain penelitian potong lintang dengan total 100 pasien tersangka KKR, terdiri dari 50 subjek KKR dan 50 subjek non-KKR yang dilakukan kolonoskopi dan pemeriksaan histopatologi. Didapatkan perbedaan bermakna nilai RNL dan RPNL pada kelompok KKR dan non-KKR. Titik potong nilai RNL untuk membedakan pasien KKR dari non-KKR adalah 3,46 dengan sensitivitas 60% dan spesifisitas 72,7%, titik potong nilai RPNL adalah 1.482 dengan sensitivitas 66% dan spesifisistas 68%. Berdasarkan hasil analisis multvariat didapatkan model kombinasi berupa sistem skoring terdiri dari parameter konstipasi kronis, RPNL, perdarahan saluran cerna, dan penurunan berat badan. Skor diatas 6 mempunyai probabilitas tinggi terjadi KKR (> 65%).

Colorectal carcinoma (CRC) is the third most common cancer and the second leading cause of cancer death in the world. The cause of death of CRC was a delay in diagnosis and treatment. Colonoscopy as a gold standard for diagnosis is invasive, costly, and uncomfortable for the patients. A chronic inflammation has a crucial role in CRC carcinogenesis. Hematology parameters such as white blood cells, neutrophil, lymphocyte, and platelets as markers of inflammation are related to progressivity of CRC. A neutrophil lymphocyte ratio (NLR) and a platelet neutrophil to lymphocyte ratio (PNLR) has been investigated by many as a prognostic marker for CRC. This study aimed to investigate the role of NLR and PNLR as parameters to detect CRC and a combination of two to predict CRC. This cross-sectional study was consisted of 100 subjects suspected of CRC and were examined of colonoscopy and histopathology, comprised of 50 CRC and 50 non-CRC. There was a significant difference of NLR and PNLR between the two groups. NLR cut-off to differentiate CRC and non-CRC was 3,46 with a sensitivity of 60% and specificity 72,7%, while PNLR cut-off was 1.482 with a sensitivity of 66% and specificity 68%. A combination model was created using the outcomes of the multivariate analysis, consisting of parameters for chronic constipation, PNLR, gastrointestinal bleeding, and weight loss. Score above 6 indicates a high probability (> 65%) of developing CRC.
